P. 28 · GUIDE

What topicals can
and cannot do.

Topicals soften, in-clinic procedures resolve.

Atrophic, rolling, ice-pick, and box-car scars require procedures, not serums. The honest version of the conversation: topical treatment helps with surrounding tone and texture, but the scar itself responds to needles, lasers, and time.

— § 01

The triggers worth tracking.

Indented texture visible in side lighting

The defining sign. If the scar is visible only in flat front-on lighting, it is more likely PIH or texture, not true atrophic scar.

94%
Rolling shadows on cheeks and temples

Most amenable to clinic resurfacing — RF microneedling and subcision combined.

78%
Ice-pick scars: deep, narrow, vertical

Hardest to treat. TCA CROSS or punch excision are the workhorse procedures.

52%
Box-car scars with sharp vertical walls

Respond well to fractional CO2 and microneedling.

45%

— § 02

Ingredients that actually work.

Tier
Molecule
Role
Evidence
Tier B
Tretinoin 0.05% nightly
Surrounding tone & texture
12-week endpoints on tone, texture, and pore quality. Will not fill the scar.
Tier B
Vitamin C 15% AM
Collagen support
Adjunctive collagen synthesis support. Helps the procedural protocol work better.
Tier A
Subcision (clinic)
Tethered rolling scars
Single most underused intervention. Cuts the fibrotic bands that anchor the scar.
Tier A
RF microneedling (clinic)
Atrophic & box-car
3–4 sessions, 4 weeks apart. The current procedural standard.
Tier A
Fractional CO2 (clinic)
Resurfacing
Most effective for box-car. Significant downtime; significant endpoints.
Tier A
TCA CROSS (clinic)
Ice-pick scars
Targeted high-strength TCA into individual ice-pick scars. Multiple sessions.

— § 03

The protocol.

  1. Phase 01 · Month 1–3

    Stabilise active acne

Scar treatment requires no active inflammation. Stabilise acne first; subcision while breakouts continue is futile.

  2. Phase 02 · Month 3–9

    Procedural protocol

Subcision + RF microneedling for rolling. CO2 for box-car. TCA CROSS for ice-pick. Combine modalities.

  3. Phase 03 · Month 9+

    Maintenance + topicals

Continue tretinoin and SPF; expect 60–80% improvement, not 100%. Photograph at month 0 and month 9.

Bottom line

Scarring is the one acne category where money does meaningfully better than discipline. Save the topical budget for procedures.
